FIRST VISIT

Forms and Paperwork

Before your first visit, our office will provide you with paperwork that needs to be filled out prior to your first appointment.  This will give Dr. Boggs a detailed account of your medical history so that he can determine how to best help you.  You will bring the completed paperwork with you to your first appointment.  We also ask that you bring in any diagnostic test and lab results that you have.

Consultation

Next, you will have a consultation with Dr. Daniel Boggs to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Daniel Boggs to learn more about you, your condition, and  expectations to determine how he can help you.

Examination

After your consultation, Dr. Daniel Boggs will perform a complete orthopedic and neurological examination.

After your consultation and examination, Dr. Boggs will review your paperwork, lab studies and examination results.  Based upon this information, he will then determine how to best help you, and also if he will accept your case.

VISIT TWO

Review Of Findings

Once the information is collected  and examinations are performed, Dr. Daniel Boggs will give you a detailed report of all findings and answer any questions including:

1. How can you help me?
2. How long will it take?
3. What  will  my treatment  cost?

We will answer all questions to your satisfaction before proceeded with care
